Application Guidelines

Thank you for your interest in the UW Division of Otolaryngology-Head and Neck Surgery. We hope this information will answer most of your questions about our program. Should you have further questions, we invite you to contact our office at (608) 265-0494 or e-mail the OTO House Staff Coordinator at oto-hns@surgery.wisc.edu.

All applications to our program should be submitted through the ERAS. Information regarding the match is available at the following website address:

To be considered for the program, please make sure your application is submitted to ERAS in time to reach our offices by the end of October of each year. The following documents must be included in your application packet to ERAS:

  • Common Application Form
  • Personal Statement
  • Medical School Transcript
  • Letters of Recommendation (3)
    Preferably from OTO Program Director or Chair
  • USMLE Transcript
  • Photograph
  • Dean's Letter

IMGS PARTICIPATING IN 2009 MATCH SHOULD REGISTER FOR STEP 2 CS NOW

Passing both parts of the Step 2 Clinical Skills examination is required to begin residency training at University of Wisconsin Hospital and Clinics. Potential applicants should not delay in registering for the Step 2 Clinical Skills examination. For more information, visit www.usmle.org and www.ecfmg.org.

All application files will be reviewed by our selection committee. We will inform you by mail or telephone of their decision regarding an interview. If you should move, it is important that you e-mail us notice of your change of address and telephone number.

The interview process usually starts in late November each year and concludes in January. This year's interview dates are:

  • December 14, 2009
  • January 8, 2010
  • January 13, 2010
