Application Guidelines

Thank you for your interest in the UW Division of Plastic and Reconstructive Surgery. We have chosen to use a centralized application/verification service that allows you to complete one application and furnish one set of documents for most of the programs in which you are interested.

For our Integrated Residency Program, please request a universal application from:

When you submit the completed application, please be certain to list the University of Wisconsin-Madison as one of the programs to which you would like your verified application sent.
