Robotic-Assisted Radical Prostatectomy

The University of Wisconsin currently offers a robotic assisted prostatectomy using the da Vinci® surgical machine. This method incorporates the latest advancements in robotic-assisted technology and allows the surgeon greater visualization, enhanced dexterity, precision, and superior control. Nationally roughly 20% of prostatectomies are now being done using the robotic approach. A growing number of institutions in the United States and Europe have embraced this technique.

The procedure requires 5 pen-sized holes. The surgeon inserts a tiny camera and different instruments all attached to the robotic arms into the patient through these holes. The da Vinci® surgical system incorporates 3 multi-joint robotic arms with one arm controlling the binocular endoscope and the other 2 controlling small wristed instruments. The forceps or scissors are remotely manipulated while the surgeon is shown the magnified images that are approximately 10 times greater than normal. This stereoscopic view of the operative field allows improved visualization. The operation is performed in a similar manner to the open radical retropubic prostatectomy procedure. The prostate specimen is then removed by enlarging one of the holes.

Advantages of this approach is a shorter hospital stay, typically overnight, less risk of infection, less blood loss and transfusions, faster recovery, and quicker return to normal activities. Urinary control, maintenance of erections and cancer control are similar to the open procedure.
